Service Description

Hospital-to-Home Care Support provides nurse-led assistance for individuals transitioning from the hospital, surgery center, birthing center, rehabilitation facility, emergency room, or care facility back to home. This service may benefit clients recovering from surgery, illness, injury, hospitalization, rehabilitation, childbirth, or a change in health condition. Support may include wellness observation, basic vital sign checks when appropriate, discharge instruction review, medication reminder concerns, mobility and safety observation, hydration and nutrition support, postpartum recovery support, caregiver education, follow-up reminders, and care coordination guidance.
